SERVICE AREA:

SERVICES AVAILABLE:
Free Initial Nursing Evaluation Visit* RNs, LPNs, CNAs, NAs, HHAs, Companions/Homemakers Hourly and Live-In.
All employees thoroughly screened, licensed and bonded.
Expertise includes:
Short Term, Long Term and Convalescent Care in the home,
Palliative care, respite care, High Tech care: TPN,
respirators/ventilators, injections,
IV hydration/antibiotics/pain management.
RN and NA Mini Visits (one hour visits) also available.
Case Management and Skilled Nursing Supervision
of all employees provided.
SERVICE MINIMUMS:
Our hourly services have a six (6) hour minimum per shift, thirty (30)
hour minimum per week; two (2) week minimum length of service.
Live-In Services have a minimum of three (3) days per week.
The Para-Professional Mini-Visit has a minimum of one (1) per week. Skilled visits do not have a weekly minimum.
When a caregiver provides care for two (2) clients in the same unit/home, it will be billed at an additional thirty five percent (35%).
METHOD OF PAYMENT:
Weekly billing directly to client.
Will also bill private insurance company, trust or guardianship account, etc.
OFFICE HOURS:
Monday-Friday, 8:30 A.M.-5:00 P.M.
24 Hour R.N., On-Call Staff, 7 days each week
OTHER:
Licensed by the Illinois Department of Public Health (IDPH)
Employees screened, orientated and supervised by the agency. Training and In-Services available to all employees.
* Initial Nursing Evaluation Visit not available for Mini-Visit services.
